Job Purpose:

The Caregiver, Medical Services will play a key role in supporting patients with daily living activities and providing compassionate, high-quality care in line with Praava Health`s standards. This role requires professionalism, empathy, and commitment to ensuring patient well-being while assisting with medical and non-medical needs.

Key Responsibility Areas:

• Assist patients with personal hygiene, including bathing, grooming, dressing, toileting, and oral care

• Provide mobility support including lifting, turning, repositioning, and transferring patients

• Support feeding and meal preparation as per dietary requirements

• Monitor vital signs and report abnormalities to the nursing supervisor/assigned physician

• Administer basic medications under supervision and as per care plan

• Visit assigned patient homes on schedule and ensure quality care and professional conduct

• Maintain accurate documentation of services provided during each visit

• Ensure all medical equipment is clean and functioning properly

• Communicate effectively with patient families and report any changes in health status

• Uphold infection prevention and control measures during all visits

• Available for 12-hour day shifts

• If no home service visits are assigned, the employee must remain present in the office
